By accessing or using the Project THRIVE website (thrivesalem.com) or any related services, you agree to be bound by these Terms of Use. If you do not agree, please do not use the site or services. If you are under 18, a parent or legal guardian must review and accept these terms on your behalf, except where self-consent is permitted by law.
Project THRIVE is a student wellness mentoring program delivered by JD Health & Wellness in partnership with Salem-Keizer Public Schools, with funding support from State Grants and participating insurance providers. The program pairs students with trained Community Health Worker mentors for weekly check-ins focused on attendance, academics, and well-being.
Project THRIVE is not a therapy, clinical treatment, or crisis service. If you or someone you know is in crisis, call or text 988 (Suicide & Crisis Lifeline) or 911.
Enrollment is open to students at participating Salem-Keizer Public Schools and to young adults in the Salem-Keizer area. Middle school enrollment requires parent or guardian consent. High school students (14 and up) and young adults may self-enroll. All information provided during enrollment must be accurate and current.
